Support Coordination (Levels 1, 2 & 3)

NDIS support coordination often breaks down quietly.

Families end up picking up the slack.
Participants have a plan but don’t feel like it’s working.
Support coordinators go quiet.
On paper, things exist, but in real life nothing is moving.

Sometimes things are technically working, but only because you’re holding everything together in your head. Appointments. Providers. Funding. Paperwork. Follow-ups. It becomes exhausting.

That’s usually when people reach out to me.


How I work

I work using a crisis, stability, then maintenance approach.

First, we stabilise what’s urgent or clearly not working.
Then we get base supports in place and make sure they actually hold.

Once things settle and you have some breathing room, we move into plan maintenance and start aligning the NDIS with your medium and longer-term goals.

I stay involved to make sure your support needs are properly documented as your life changes, so the NDIS works alongside you as you grow, rather than lagging behind or needing constant correction.

Common situations

  • You have an NDIS plan, but it doesn’t reflect what life looks like now
  • Supports exist but nothing is flowing, or the system keeps stalling
  • A change has happened and you are not sure how to update the NDIS properly
  • You are managing for now, but it is taking too much energy to keep everything afloat
  • You can see problems coming and want to stabilise things before it becomes a crisis
  • You are helping a family member and it is becoming too much to carry alone
  • You’ve had support coordination before, but it didn’t help or didn’t last

My role is to do that work quietly and properly, so progress is real and sustainable, not just talked about.
